Steps to Reproduce:

• Log in to Vitagist.
• Click on the Health module at the bottom of the tab.
• Click on the + button and select Add Family History.
• Select a Condition Name and Relationship from the dropdowns.
• Click Cancel to close the form.
• Click the + button again and select Add Family History.
• Observe that the previously selected appear in Relationship is still populated.

Expected Result:
The form should be cleared when reopened. Previously selected values should not be retained.

Note : This issue applicable for Create the Family history and save it. When adding the new one ,Observe that the previously selected appear in Relationship is still populated.
