Heath-Health Info [Allergies ] - UI Issues

Description
1. Login the vitagist
2. Click on Health
3. Click on Health Info
4. Click on Allergies
- Headings and Icons should be changed as per design
- Input lines are appearing twice. One line should be removed.
- Severity drop down according to the design.
