Description
1. Login the vitagist
2. Click on Health
3. Click on Health Info
4. Click on Allergies
- Headings and Icons should be changed as per design
- Input lines are appearing twice. One line should be removed.
- Severity drop down according to the design.

1. In Allergies page, icons of each field should appear at the same line [comment field icon should slightly move to right side. rest others are displaying at the same line] - Image -1
2. Hint text of every field should be displayed at the same line - Image - 2
3. General Info heading should be removed. - Image -3
