Steps to Reproduce:
* Log in to Vitagist.
* Go to Health > Apps & Devices.
* Select any device.
* Click the (+) button located at the bottom right.
Expected Behavior:
A new entry screen should appear.
All fields should be empty by default.
Actual Behavior:
Existing entries appear, but some fields are pre-filled.
Note:
This issue is observed across all devices in the Apps & Devices section.
