Care Plan:- There is no back button for Care plan page. Once click on care plan

Steps to Reproduce:

  1. Log in using the Vitagist account.
  2. Navigate to the Health module.
  3. Select the Care Plan option.
  4. Click on any Care Plan item.
  5. The Care Plan Add page will appear.
  6. Notice that there is no "Back" button on this page.
  7. Please refer to the attached screenshot for reference.

Expected Result:
A " < " button should be implemented on the Care Plan Add page, consistent with other health module entries.
