Care Plan – Add Diet editing issue with appended entries

Description
Description: When editing “Add Diet” entries, recently added entries are automatically appended to the existing values, making it appear as though the maximum limit has already been reached.
Steps:-
1.Login with Vitagist
2.Go to the Health module
3.Click on Care plan
4.Edit the Diet Details and save it
5.Again edit the Diet plans
6.There Diet plan values should be newly updated
7.However, recently added values are updated on existing entries.
