openCreate Matter [Legal/General] : The document gets added automatically without clicking the "Add" button.

Description
Steps to Reproduce:
1. Log in to DGC
2. Open the menu
3. Navigate to Matter and select either Legal or General
4. Click the Create tab
5. Enter the required details and click Save
6. Select the department and click add and click on save
7. Select a document but do not click the "Add" button
8. Click on the "circle" (refresh or back) for Department
9. Then click on the "circle" for Document
Actual Result:
The previously selected document is added automatically without clicking the "Add" button.
Expected Result:
The document should only be added when the "Add" button is explicitly clicked.
Note : This issue applicable for View Matter - View Details as well.
