openCreate Matter [Legal / General] : Documents Auto-Added Without Clicking "Add" in the document page.

Description
Steps to Reproduce:
1. Log in to Lauditor using any TM or GH account.
2. Go to the Matter tab from the menu.
3. Select Legal / General Matter.
4. Click on the Create tab, fill in all mandatory fields, and click Save.
5. Select Client A and Group A, then click Add.
6. Go to the Documents section.
7. Select multiple documents, then unselect any one of them.
Expected Result: No documents should be added unless the Add button is explicitly clicked.
Actual Result: Documents are added automatically after selection changes, without clicking Add.
Note: This issue occurs on both Create and View screens for Matter [Legal / General].
Note: Even after removing the document, it remains selected in the dropdown, and its label is still displayed in the 'Add Document' dropdown
