Documents (Upload):- When trying to upload a health document, the metadata edit options are not available after selecting the document.

Description

Steps:-
1.Login with Vitagist
2.Go to the Documents module
3.Select the Category as Health
4.Browse the document and upload it
5.Click on next button
6.Click on upload button
7.There invalid warning message will appear
8.'Description fields is required'

Expected: After selecting the document, the document name and description edit options should be available
