Description

Steps to Reproduce:

1. Login to the iOS Vitagist app.
2. Go to the Connections section.
3. Tap Add Contact.

Issue:
The following fields should be removed from the Add Contact form:

  • Relationship / Account ID
  • Entry Identification Number

Expected Result:
These fields should not appear in the form.
