Hospital Side - Audit trails - When the hospital receives a complaint, should update the audit records

Description

1. Login to the patient app
2. Raise the complaint from any department
3. After raising the complaint login to the hospital app with AAM / DH / SU
4. Click on Audit trails

When the hospital receives a complaint, should update the audit records in the following format

Case - 1 : If only the ward no is entered, New Complaint: Patient in ward no <ward no.> has raised a complaint. Department: <Department Name>; Issues: <Issues selected>

Case - 2 : If the both ward no and name are entered, New Complaint: Patient <Name> in ward no <ward no.> has raised a complaint. Department: <Department Name>; Issues: <Issues selected>
