Health - Health Info - In both Immunization and insurance, primary country only be listed in Document Upload Page, secondary country also should be listed while uploading the documents

Description

1. Login the VitaGist
2. Click on Health
3. Click on Health Info
4. Select the Insurance/ Immunization that has been already added
5. Click on document upload icon
6. In Document Upload page, primary country only be listed

All secondary countries that are added in country affiliation section also should be listed while uploading the document for Insurance and Immunization
